Listen and save. Start by auscultating their abdomen. As soon as the head touches the child, make an exaggerated surprised look on your face. Do this each time you auscultate. This has the effect of drawing the child's attention away from the stethoscope (and perhaps trying to push it away) as well as entertaining them.
